Caretaker Contract

This contract outlines the terms of employment for a caretaker, detailing their responsibilities for patient care, household duties, and transportation. It specifies working hours, payment rates, and requirements such as maintaining a valid driver's license. The agreement is between the Caretaker and a Client, who may also be the Patient.

Caretaker Contract

[name of caretaker] (hereafter “Caretaker”) and [name of client] (hereafter “Client”), hereby agree to the following contract regarding the care of [name of patient], if different from client (hereafter “Patient”):

 

The Caretaker shall be employed no less than [number] hours and no more than [number 1] hours per week. The number of hours per week may fluctuate based on the Patient’s needs and will be finalized no fewer than [number 2] days in advance.

The Caretaker’s responsibilities regarding patient care shall be: [medication administration, bathing, feeding, bathroom, supporting/carrying duties].

The Caretaker’s household responsibilities shall be: [bill pay/organization, meal prep, plant watering, lawncare, cleaning duties].

The Caretaker’s transportation responsibilities shall be: [shopping, errands, doctor visits, appointments, church, etc. duties].

The Caretaker shall maintain a current driver’s license in [state] and a clean driving record. The [caretaker/client] will provide the vehicle for transportation. The [caretaker/client 1] will pay for transportation costs, including [fuel/maintenance].

The Caretaker will be paid an hourly rate of [amount] and will receive paychecks [monthly/bi-monthly/weekly].

 

In witness to their agreement to the terms of this contract, the parties affix their signatures below:
