MODEL RELEASE FORM
I, [model name], hereby grant [photographer name] and their representatives, successors, and assigns the irrevocable and unrestricted right to use, reproduce, and publish photographs of me, taken on [date] at [location], for any lawful purpose, including but not limited to advertising, trade, or any commercial or artistic use, in any medium now known or later developed.
I acknowledge that [photographer name] owns the copyright in these photographs and I hereby waive any claims I may have based on any usage of the photographs or works derived therefrom, including but not limited to claims for invasion of privacy, right of publicity, or defamation.
I represent and warrant that I am at least 18 years of age, have the full legal capacity to execute this agreement, and have read and fully understand the contents of this release. This agreement shall be binding upon me, my heirs, legal representatives, and assigns.
Option 1: I will receive [compensation amount] as full compensation for this release and all rights granted hereunder.
Option 2: I acknowledge that I will receive no compensation for this release and all rights granted hereunder.
