This template provides a set of four forms designed for parents to manage Shared Parental Leave (SPL) and Shared Parental Pay (ShPP). These forms facilitate the curtailment of maternity leave and pay, and notify employers of the intention to take SPL, whether by the mother/birth parent, their partner, or both.
