About this template
What is this template?
This is a sworn legal statement used in India to bridge the gap between an old name and a new name for administrative record-keeping. It is not a Deed Poll or a Gazette Notification, which are separate steps usually required before or alongside this affidavit. It specifically contains a verification clause where the deponent swears to the truth of the identity claim under penalty of perjury.
When should you use it?
Use this document when an educational institution or board requires formal proof of a name change to issue a corrected certificate. If you are changing your name for general purposes and have not yet published it in the Gazette, use a Name Change Deed Poll instead.
What's inside
| Clause | Name | What it does |
|---|---|---|
| 1 | Identification of Deponent | The Deponent provides their current name, age, and residential address to establish their legal identity for the record. |
| 2 | Declaration of Name Discrepancy | The document specifies both the old name as it appears in certificates and the new name intended for future use. |
| 3 | Consistency of Identity | The Deponent affirms that the person named in the existing certificates and the person with the new name are the same individual. |
| 4 | Purpose of Affidavit | The statement declares that the affidavit is being executed specifically to request a name change in the specified certificate records. |
| Verification | Verification | The Deponent swears that the contents are true and no material facts have been concealed, preventing future disputes. |
Who it's for
