PERSONAL TRAINING CONTRACT
This Personal Training Contract (hereinafter referred to as the "Agreement") is made and effective [Effective Date] (the “Effective Date”),

SERVICES
The Trainer will provide the following services to the Client:
The personal training sessions conducted by the Trainer will have a duration of ________ minutes/hours (choose one) per session.
SESSIONS
The training sessions conducted by the Trainer will include the following activities:
PAYMENTS
The Parties agree that the Client will purchase ____________ training sessions at a rate of ____________ per session.
The Client may choose to pay for each session individually or make a monthly payment in advance at a discounted rate of ____________ for all the training sessions.
CANCELLATION
The Client acknowledges and agrees that it is their responsibility to notify the Trainer at least 24 hours in advance in the event of a cancellation. Failure to do so will result in the Client being charged the full rate for the cancelled/missed training session.
The Trainer also agrees that it is their responsibility to notify the Client at least 24 hours in advance in the event of a cancellation.
INDEMNITY
The Client agrees to hold the Trainer harmless in the event of any injuries or illnesses experienced during the training sessions.
WARRANTIES
The Parties acknowledge that the training program organized by the Trainer is designed solely for the benefit of the Client's health and wellness.
Furthermore, the Parties agree that while the Trainer does not guarantee specific results from the training sessions, they guarantee to exert their maximum effort during the training sessions.
TERM
This Agreement shall be effective as of the date of signing (the "Effective Date") and will continue until _____________________________.
Upon the expiration of the Agreement term, it will not be automatically renewed for a new term.
TERMINATION
This Agreement may be terminated under the following circumstances:
Immediate termination in the event of a breach of this Agreement by either Party.

DISPUTE RESOLUTION
