This agreement is made on [date] between [name of band representative], hereafter “Band,” and [name of couple], hereafter “Couple,” regarding the contract of a music performance for [date 1] at [reception venue]:
The Band will provide [number] hours of musical service at [location] ONLY, plus [number 1] hours for set up and clean up. Any overtime requests or venue changes will be charged [rate].
