This is an official template for Form TM-P under the Indian Trade Marks Act, 1999, used to request post-registration changes to a trademark. It covers various updates including amendments to proprietor details, assignment of ownership, cancellation of registration, and changes to the trademark or goods/services description.

The Trade Marks Act, 1999 Application for Post registration changes in the trademarks

This form is divided into three Parts i.e. A, B & C. Part A and C are required to be filled compulsorily. Part B is further divided into several subcategories and the same is required to be chosen by the applicant as per requirement and be filled accordingly. [Note: One form is meant for one request only. Different forms should be submitted for different requests. In case more than one request is claimed in one form the first one will be entertained and rest of the requests will be ignored.] [(a) One form is meant for request of one change only. Different forms should be filled up for different changes (b) The relevant information must be filled up in colored box against the respective head.]
