UK Adult Model Release Form
Purpose: This form grants a photographer or “Artist” the right to use photographs, video and sound recordings of a model who is over 18 years of age. Under the UK GDPR and Data Protection Act 2018, images of individuals constitute personal data and require consent for processing. A UK model release should specify how personal data will be used, confirm that the model has been informed of their data protection rights and include a privacy statement or reference to a privacy policy.

Grant of Rights
For good and valuable consideration (the receipt of which is acknowledged), the Model grants the Artist and the Artist’s licensees and assigns the exclusive permission to use the Model’s image and likeness, as captured in photographs, video, and/or sound recordings (the “Media”), in any media and for any purpose, including advertising, promotion, marketing and packaging for any product or service. The Model agrees that the Media may be combined with other images, text and graphics, cropped, altered and modified, and hereby releases the Artist from any liability related to the use or licensing of the Media
Privacy and Data Protection
The Model acknowledges that the Artist will process their personal data (including images and contact information) in accordance with the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018. The personal data collected will be used solely for the purposes specified in this Release and any related marketing or promotional activities.
The Model understands that:
Lawful Basis: Their consent forms the legal basis for processing their personal data.
Data Subject Rights: They have the right to access, rectify, erase or restrict processing of their personal data, and to object to processing.
Withdrawal of Consent: They may withdraw consent at any time by contacting the Artist in writing. Withdrawal will not affect any use of the Media that has already been made but will prevent further use of the Model’s image in future publications.
Data Retention: The Artist will store the Media and personal data securely and retain them for as long as necessary for the purposes described herein, unless the Model requests erasure sooner.
Waiver of Inspection and Approval
The Model waives the right to inspect or approve the finished Media or any advertising copy or printed matter that may be used in connection with them, as well as the right to seek compensation other than as agreed herein.
Release of Liability
The Model releases and forever discharges the Artist, the Artist’s legal representatives and assigns, and all persons acting under the Artist’s permission or authority from any and all liability and claims arising out of or relating to the use of the Media, including any distortion, alteration or composite use, and any claims for libel, invasion of privacy or other rights.
Warranties and Age
The Model warrants that they are over 18 years of age and have full legal capacity to execute this Release. The Model has read this Release and fully understands its contents.
Governing Law
This Release and any dispute or claim arising out of or in connection with it shall be governed by and construed in accordance with the laws of England and Wales. The parties submit to the exclusive jurisdiction of the courts of England and Wales.
