This template is a formal letter for an employee to appeal a disciplinary penalty issued by their employer. It allows the employee to clearly state their reasons for the appeal, such as disagreement with the process or outcome, or the introduction of new evidence. The letter also requests a meeting to discuss the appeal and allows the employee to indicate if they wish to be accompanied.
