Order Form for Cloud Service Agreement (CSA)

Subscription details
Pilot | This is an optional Variable. If there is no trial or pilot, delete this entire row. Customer may access the Cloud Service for a limited [pilot period] trial ("Pilot Period"). The Subscription Period will automatically start following the Pilot Period. If including a pilot, select one option. Additions and Modifications

If Provider does not meet the SLA in two consecutive months or over three months in any 12-month period, then Customer may, as its only remedy, terminate this Order Form upon notice and receive a prorated refund of prepaid Fees for the remainder of the Subscription Period.] |
Professional Services | If providing professional services, use the first option to incorporate an existing SOW or professional services agreement or the second option to describe the professional services on this order form.
