About this template
What is this template?
This document is a formal sworn statement submitted to a University Registrar to request a name update on academic records. It is not a marriage certificate or a general deed poll, but a specific declaration of identity continuity between a maiden and married name. It contains specific fields for the degree roll number, graduation years, and the husband's details.
When should you use it?
Use this when a university requires a formal affidavit to issue a degree certificate in a married name. Use a Deed Poll or Gazette Notification if you are changing your name for reasons other than marriage or for general purposes.
