Data Use Agreement

This Data Use Agreement outlines the terms under which a Data Provider will share specific data with a Data Collector. It details the scope of the data, including a limited dataset that excludes identifying factors, and establishes restrictions on how the Data Collector and any third-party recipients may use and protect this information. The agreement also includes provisions for notifying parties in case of security breaches or pertinent discoveries.

Data Use Agreement

 

[name of provider] (hereafter “Data Provider”) and [name of corrector](hereafter “Data Collector”) hereby agree to the following:

 

The Data Provider agrees to provide the Data Collector with the following: [enter data information] (hereafter “the Data”).

The Data Provider hereby waives all rights to access the Data and agrees not to make or provide copies of the Data to any third parties.

The Data Collector will have access to the information collected as outlined above as well as a limited dataset (hereafter “Limited Dataset”) including the following: [dataset such as age, dates, city, etc.]. The dataset will NOT include any identifying factors, including the following: [{facial photographs, name, address, contact information, social security number, account numbers, etc.}].

The Data Collector may share the Data as well as the Limited Dataset with the following recipient(s), as well as their agents, representatives, and subcontractors (hereafter “The Recipients”): [recipient(s)].

All recipients, as well as their agents, representatives, and subcontractors, will abide by the following restrictions:

The Recipients will not contact the Data Provider for any reason. If a discovery is made using the Data or Limited Dataset that is pertinent to the Data Provider for a security, educational, financial, medical reason, the Recipients will notify the Data Collector of the information.

The Recipients will not disclose the Data or Limited Dataset to any outside persons or organizations for any reason.

The Recipients will have reasonable safeguards in place to protect the Data and Limited Dataset from being stolen or accessed by outside persons or organizations.

The Recipients will notify the Data Collector if there is a security breach regarding the Data or Limited Dataset.

The Recipients’ use of the Data and the Limited Dataset is restricted to [usage].

 

The Data Collector agrees to notify the Data Provider of any urgent information or security breaches once notified of such by the Recipients.
