This General Affidavit template allows an individual, the Affiant, to formally declare facts under oath or affirmation. It includes sections for personal details, the purpose of the affidavit, a statement of facts, and a concluding affirmation under penalty of perjury. The document also provides a section for a Notary Acknowledgment, which can be omitted if not required.
